Do i need a transmission fluid change or something.?

My Lincoln Ls 2003's transmission revs to 3 RPM when on 2RPM, while shifting to 2nd to 3rd gear. it is automatic, and it also has a semi-manual.Its a v8 5 speed. at 96 thousand mileage.. once it revs it forces the transmission change, and makes a hard shift. I already had the spark plugs change, so that couldn't be the problem.
